At the brink refers to her being so close to going over that it would only take a stroke or two to reach the point of no return. Getting there builds up a lot of tension, and keeping her there for long periods, by changing stimulation just enough to not let her go over several times as she approaches a climax, can build up a large amount, resulting in a huge release when she finally does go over.
